We look forward to meeting you! Here are a few things to help you prepare for your first visit with us:
After making your appointment, you will receive an email from Living Matrix, the functional medicine electronic medical record, to help you prepare for your visit. This email will have links and instructions to guide you in completing several detailed health forms before your visit.
At least one week prior to your first appointment, please email or fax any medical records, lab or imaging results you have from the past 2 years. Email to drterry@balancedintegrativehealth.com, or fax to 515-639-3803. If you are unable to email or fax records, please bring them to your appointment.
Download and complete the patient Authorization and Consent forms (see the link below). Please fax, email or bring these forms to your first visit.
Bring the following to your first appointment:
- Questions and issues you want to discuss
- Your current supplement and prescription bottles
- Medical records not sent before your appointment
- Notepad or device for taking notes
Payment Options:
Balanced Integrative Health only accepts cash, debit and credit cards including HSA and flexible spending cards. We do NOT accept health insurance or Medicare. To read more about payment, click HERE.
Cancellation of appointments:
If you need to cancel or re-schedule your appointment, please do so within 3 business days of your appointment, via voicemail or email. When a patient cancels within 48 business hours of an appointment or is a “no show,” we miss the opportunity to provide care to other patients during that time slot.
